BRISBANE TO SUNSHINE COAST

Based on the North Side of Brisbane, Rise Occupational Therapy provides in-home & community based services across Brisbane through to the Sunshine Coast.

Occupational Therapy Driving Assessments

Driving assessments are completed by an Occupational Therapist who has undertaken postgraduate studies in this area. Driving Assessments assist in determining the impact of any medical conditions or disabilities on fitness to drive, and make recommendations for vehicle purchase and modification. The aim of a driving assessment is to facilitate safe and independent vehicle access and driving.

Our driving assessments consist of an off-road and an on-road component, which can be completed from home and in your local area.

Equipment Prescription

ADL Equipment to allow independent and safe completion of daily tasks including self-care, cooking, cleaning, and dressing.
Mobility equipment recommendations
wheelchair prescription and seating reviews

Applications for equipment including MASS, CAEATI, and DVA requests (see Referral Options).

Home Assessments

Home assessment services are available to ensure the home is safe and accessible, despite changes in physical or cognitive functioning.

  • Home access and modification assessments

  • Carer support and training, including manual handling

PAEDIATRIC SERVICES

Standard Initial Assessment

  • 80-minute session including parent interview and assessment using standardised tests and clinical observations
  • Tailored to meet the requirements of your referral, as determined through referral letters and initial phone conversation
  • Provision of a comprehensive written report outlining findings and recommendations

Extended Initial Assessment

  • 80-minute session including parent interview and assessment using standardised tests and clinical observations
  • 40 minute feedback session with parent / caregiver to discuss assessment findings and develop therapy goals
  • Tailored to meet the requirements of your referral, as determined through referral letters and initial phone conversation
  • Provision of a comprehensive written report outlining findings and recommendations

Individual Therapy Session (35 mins)

  • 30-minute session utilising child-centred activities that target individual therapy goals
  • Play and function based therapy tailored to the individual child’s needs and interests
  • 5 minutes allocated at the end of the session to discuss progress, home based activities, parent concerns

Additional Documentation

Includes but not limited to:

  • Therapy outlines / goal summaries
  • Progress reports and letters
  • Letters to Paediatricians/treating GP/other allied health professionals
  • Details of recommendations for schools and teachers
  • Comprehensive home programs
