Rise OT is Brisbane based and provides Occupational Therapy (OT) services for adolescents and adults. Our service was established in 2015 by two Occupational Therapists who saw an opportunity to provide quality services in their local community. Since this time, we have continued to grow our scope of practice based on the specialised skills of our team. We are a small team of experienced Occupational Therapists who love what we do. We have a supportive practice and close knit workplace, enabling us to ensure we are providing quality interventions. We value multi-disciplinary practice and love working with other allied health professionals and service providers to ensure optimal outcomes for our clients.

BRISBANE TO SUNSHINE COAST

Based on the North Side of Brisbane, Rise Occupational Therapy provides in-home & community based services across Brisbane through to the Sunshine Coast.

Occupational Therapy Driving Assessments

Driving assessments are completed by an Occupational Therapist who has undertaken postgraduate studies in this area. Driving Assessments assist in determining the impact of any medical conditions or disabilities on fitness to drive, and make recommendations for vehicle purchase and modification. The aim of a driving assessment is to facilitate safe and independent vehicle access and driving.

Our driving assessments consist of an off-road and an on-road component, which can be completed from home and in your local area.

Equipment Prescription

ADL Equipment to allow independent and safe completion of daily tasks including self-care, cooking, cleaning, and dressing.
Mobility equipment recommendations
wheelchair prescription and seating reviews

Applications for equipment including MASS, NDIS, and DVA requests (see Referral Options).

Home Assessments

Home assessment services are available to ensure the home is safe and accessible, despite changes in physical or cognitive functioning.

  • Home access and modification assessments

  • Carer support and training, including manual handling
  • Case management

NDIS

Our NDIS funded OT services are provided under the “Improved Daily Living” budget within NDIS plans. We can provide services to self-managed, plan managed, and agency managed participants. Our NDIS services include, but are not limited to:

  • Prescription of Assistive Technology (AT) Including Assessment & Trial
  • Completion & Submission of AT Applications
  • Home Assessments for Both Minor and Major Home Modifications
  • Completion of Complex Home Modifications Applications
  • Driving Assessments and Training
  • Prescription of Driver and Passenger Vehicle Modifications
  • Completion of Functional Assessments to Inform Plan Reviews
  • Supported Independent Living (SIL) Assessments
  • Specialist Disability Accommodation (SDA) Assessments
  • Individual OT Appointments and Interventions As Required, Based on Referral Information

We are also able to assist individuals interested in becoming NDIS participants through completion of functional assessments and assistance with goal setting. The reports provided from these assessments can be utilised to inform NDIS applications and planning meetings.

Telehealth

Telehealth appointments are offered to eligible clients. We utilise video and phone technology to deliver OT services. Telehealth services are supported by many funding bodies. An initial intake phone conversation will be conducted to ensure these services are the most appropriate option to meet client needs prior to commencement of intervention.

Telehealth and video conferencing provide an effective method of delivering OT services without requiring face-to-face appointments. They allow increased infection control and access to supports for clients located in isolated areas. To participate in Telehealth services you will require an internet connection and a suitable device with a camera and a microphone (eg. tablet, iPad, laptop, smart phone). Prior to commencement of Telehealth services, we will assist you with set-up through provision of easy to follow instructions. We provide an appointment time and link to join the session via email to allow ease of access to services.
